We actually covered this in my on-ship lessons... I have no idea WHY, but we did. (Not that I could remember, so I had to look)
It's the same way you help someone who's got fat in the way of a normal heimlich-- position your hands just *above* the joining of the lowest ribs. (Or, if you're doing an emergency heimlich on yourself, throw yourself against a counter with your fists just above that really painful point where any really comfortable bra is now rubbing red...if my experiences were any guide. ;^p)
I asked this question too once at a CPR class, and the basic answer is like what you already got. You've got to risk hurting the baby, because otherwise everyone is going to die!!!
This is how I went into labor... but I don't recommend it as an induction method. ;)
Seriously, I choked and coughed and hacked so hard my water broke. I was past my due date, so it was OK. My poor mother was terrified she'd have to try the Heimlich, though. She hasn't made the same pasta salad since, just in case!

That's why I only ever execute the Heineken Maneuver myself ...

Chest thrusts, rather than abdominal; if she's in a full choke she won't be long falling down anyway, so go straight for compression only CPR.
